Disease or Disorder Report
Each student will present an ORAL Report and a 3 page TYPED Report (title page, minimum one half page summary, and a two source reference page). Computers are available in the library and career center during lunch, and before and after school. This assignment is worth 50 points, 25 points for the oral presentation and 25 for the typed portion. The typed portion will be graded using the Guilford High School writing rubrics. No handwritten reports or late reports accepted.
The following information to be included on the oral and
typed reports for a disease or disorder:
1.
Name of the disease or disorder and why you selected it.
2.
How is the disease or disorder transmitted?
3.
What are the symptoms?
4.
Is there a cure? If so, what is the cure? If there isn’t a cure, how
are the symptoms treated?
5.
Additional information: How many people have the disease? Who is at
risk for the disease? Can you prevent the disease?

Oral Reports (25 POINTS)
The oral presentation will be graded on answering the 5 questions (5 points an answer). DO NOT READ YOUR SUMMARY. REFER TO INDEX CARDS. (I suggest one index card per question.)

Typed Reports (25 points): Typed reports must be typed. No handwritten or late reports will be accepted. The typed report should consist of three pages 1) title page (disease/disorder name, your first and last name, Health, date, hour), 2) minimum one half page summary, with paragraphs answering the five questions listed above (single or double space, 1.25 margins, 12 font), and 3) two references (follow guidelines below). The typed portion will be graded using the GHS writing rubrics.
